**Summary**:
A Home Health Scheduler/Care Coordinator is the primary gatekeeper, liaising between the Home Health Company, patients, and the medical providers. Your primary function is to describe all the company services to the patients fluidly and accurately, answer all their inquiries, identify the available provider near the patient based on their zip code, and ensure that quality service and patient satisfaction are met. The Scheduler/Care Coordinator must manage and coordinate patient visits (if any), matching the schedule with the medical staff availability within the area of the patient and suitability on what kind of patient care service is needed.
**Roles and Responsibilities**:
- Manage incoming and outgoing calls, conduct insurance verification procedures, and arrange appointments effectively.
- Explain in detail all the relevant services the provider offers their patients and determine the most suitable.
- Responsible for creating and maintaining home health professionals' daily, weekly, and monthly schedules.
- Coordinate with clients and their families to schedule appointments and visits that are convenient for them. This may involve assessing client preferences, availability, and special requests.
- Collaborate with nurses, therapists, and home health aides to determine their availability and preferences for patient assignments. Ensure that the right healthcare professionals are matched with patients based on their needs and qualifications.
- Optimize schedules to enhance the efficiency of care delivery, which may include minimizing transit time between patient visits and mitigating avoidable delays.
- Maintain effective communication with clients and healthcare providers to address scheduling concerns, delays, or special requirements. Provide timely information about schedule changes.
- Perform other essential duties as assigned or required.
**Job Requirements**:
- Extensive collaboration and customer service are required for both internal and external stakeholders.
- A minimum of 1 recent year of scheduling and clinic intake experience in a home health context is mandatory.
- Proficiency in utilizing tools like Google Maps to identify available providers near the patient's location.
- Exceptional courtesy and communication professionalism are vital for all parties' interactions.
- Strong organizational and clerical skills are essential for effective record-keeping and scheduling responsibilities.
- A background in nursing or a related field with knowledge of medical terminology is preferred.
- Building and maintaining relationships with staff, clients, families, and referral sources is crucial.
- Proficiency in working collaboratively within a team and managing tasks with mínimal supervision.
